Health Room Services
The goal of school health services is to keep students healthy, safe and ready to learn.

Required Medical Examinations for School Entry

·       Please share all health conditions that may interfere with your child’s academic learning and physical requirements

·       Physical and dental examinations should be completed and turned in to the school nurse every year.

·       Physicals must be current, no later than 4 months prior to the upcoming school year (May 1st)

·       Sports Physical must be no later than June 1 of the upcoming school year

·       Health and/or action plans for students with health conditions should be provided

School Immunization Requirements for School Entry
For Attendance the following vaccinations are required:
All Grades

·       4 doses of tetanus, diphtheria and acellular pertussis* (1 dose on or after the 4th birthday)

·       4 doses of polio (4th dose on or after 4th birthday and at least 6 months after previous dose given) **

·       2 doses of measles, mumps, rubella ***

·       3 doses of hepatitis B

·       2 doses of varicella (chickenpox) or evidence of immunity

·       Usually given as DTP or DTaP or DT or Td

o   **A 4th dose not necessary if the 3rd dose was administered at age 4 years or older and

o   at least 6 months after the previous dose

o   *** Usually given as MMR

7th Grade
·       1 dose of tetanus, diphtheria, acellular pertussis (Tdap) on the first day of 7th grade

·       1 dose of meningococcal conjugate vaccine (MCV) on the first day of 7th grade

MEDICATION
·       All medications must be administered by a licensed health professional unless other school personal are trained to do so.

·       Medications should be given at home whenever possible, however if this is not possible a medication order from a qualified healthcare provider must be provided.

·       The medication must be in its original container from the pharmacy labelled with your child’s name, name of medication, dose, route and prescribing doctor/practitioner.

·       Pain medication for occasional pain relief specifically Acetaminophen (generic Tylenol) for headaches and ibuprofen (generic Motrin) for menstrual cramps are on hand as needed, however chronic conditions such as migraines and dysmenorrhea (severe menstrual cramps) need a medication order from a qualified healthcare provider.
** Every year parents must fill out and sign an S-865 form for Tylenol or Motrin to be given to your child by the nurse. Please print and return this form.

·       Emergency medications such as Inhalers, Epi-pens, Diastat, Glucagon should be provided with MD orders and Action Plans.

State Mandated Screening are done by the nurse and designated health care professionals that come to our school.
·       Height/Weight (BMI) – Annually all grades

·       Vision Acuity – Annually all grades

·       Hearing – Grades Kindergarten, 1st, 2nd, 3rd, 7th

When Should a Student Stay Home from School?
For the prevention and control of communicable disease

Before returning to school:
·       Students should be fever free for 24 hours without use of fever reducing medications

·       Students should remain at home until 24 hours after the last episode of vomiting and/or diarrhea

·       Students should be free of constant/continuous coughing

·       Students should remain home for 24 hours after taking the first dose of an antibiotic

Upon return please provide an excuse note to the office for your child.
